All of the following completed forms are needed before your child can begin classes.  Please call our office at 713-780-1299 for availability, or come by our office between 9:00 AM - 5:00 PM for a tour.

2012 - 2013 School Year

  

    1.  A completed enrollment form

          2.  A registration fee (non-refundable)

                       all ages          $230.00

    3.  A deposit fee

                $200.00 one-time fee to be held until your child leaves our program.

    4.  A copy of your child's birth certificate

    5.  A completed immunization record form

    6.  A completed health requirement form

    7.  A completed, notarized emergency release form

 

2012 Summer

 

    1.  A completed enrollment form

          2.  A registration fee (non-refundable)

                       all ages          $ 75.00

    3.  A deposit fee

                $250.00 (non-refundable - applied to July tuition)

    4.  A copy of your child's birth certificate

    5.  A completed immunization record form
